 Notice!  The application deadline was prolonged.  You can send the application form by June 15, 2005.

APPLICATION  FORM

You are advised to send the completed Application form to the school organisers by May 31, 2005 in order to reserve your place in the Summer School. After that date, please fax or e-mail for any possible vacancies. All students who need a visa are advised to apply at least 3 months before the ISLS commencement.




FEES

Please notice that the below mentioned fee is in CZK. See the actual exchange rate for payment in foreign currency at the Czech National Bank. The payment in foreign currency must be equivalent to the amount in CZK and all bank charges related to the transfer must be borne by the payer.

Individual Student	26 000,-
Member of a group of 10	22 000,-
Loyalty Fee for Individual Student  (from 3rd visit onwards)	16 900,-
Individual scholarships holders*	16 900,-

* Individual scholarships are available to students from Eastern Europe and developing countries. Requests for scholarships must be submitted as early as possible and are subject to individual approval.

All fees include the following items:

• tuition (92 teaching hours)
• provision of course materials
• accommodation and breakfasts
• 3 whole day and 3 half day excursions
• afternoon and evening programmes

The fees do not include:

• travel to/from Pilsen
• health insurance and visa charges
• lunches and dinners

ATTENDANCE CERTIFICATE

All participats will receive attendance certificates. They will contain the amount of hours and the level of course attended.
